PERSONAL ARTICLES FORM DECLARATIONS PAGE

AAIS provides advisory declarations for this coverage. Insurance carriers can use the forms in part or in whole. However, the information on the declarations is necessary because of references to them within the coverage form. Two different declarations are used with this coverage.

PM 0050–INLAND MARINE DECLARATIONS

The following information is displayed on the Declarations:

· The name and address of the insurer (with or without a company logo)

· Policy number

· Name and mailing address of the agency

· Name and mailing address of the insured

· Residence location but only when different from the mailing address

· Policy period including effective and expiration date with month, day and year, as well as time of day

· Loss payable information.

Note: This is particularly important because cancellation and non-renewal notices must be mailed to any loss payable.

· A listing of all coverage parts and endorsements that are part of the policy at inception must be provided.

PM 0051–SUPPLEMENTAL DECLARATIONS–PERSONAL ARTICLES COVERAGE

This form supplements the PM 0050, allowing the named insured to specify the classes of property and coverage method desired for each, selected class.

Scheduled Coverage is available for bikes, cameras, coins, fine arts, furs, golf equipment, jewelry, musical instruments, silverware and stamps. Unscheduled coverage is also available for these classes with the important exceptions of furs and jewelry. Further, sublimits apply to coin collections and stamps.

Space for indicating whether separate schedules must be referenced is provided for personal articles, jewelry or fine arts. A listing of deductibles is available for all classes referenced above. However, deductibles are not applicable to coin collections, furs, silverware or stamps. The form also provides space for listing the locations where furs are kept.

Coverage Extensions

This section explains the extended protection for newly acquired fine arts and new acquisitions involving cameras, musical instruments, jewelry and furs.

Optional Coverages and Premium Credits

This section provides space for selecting whether any of the following apply:

• Safe/Vault Credit Coin Collections
• Safe/Vault Credit Stamps
• Vault Restriction and Credit (Jewelry)
• Named Perils Coverage (Musical Instruments)
• Breakage Coverage (Fine Arts)
• Additional Person Insured for Ring (Jewelry)

Several selections require additional information, such as storage location.
